What You’ll Need to Get Started
For clients with Empire Plan (NYSHIP) or AETNA coverage, please provide:

● A prescription from a qualified healthcare provider (MD, PA, or Chiropractor) that includes:
● Your full name and date of birth
● A diagnosis code or written diagnosis
● A prescription specifically stating Massage Therapy (not Physical Therapy)
● Start and end dates for treatment
● Provider’s contact details and signature

● A copy of your insurance card (front and back)
